Your answers will be imported into the OHSE 007–Safe Work Method Statement (SWMS) document template sourced and adapted from WorkSafe WA Subby Pack and emailed to you for review within 10 working days. 

While Prompt Safety Solutions strive to capture the scope submitted in this form, the resulting draft SWMS it is not conclusive and is the responsibility of the PCBU to implement, maintain and review with workers.

Organisation/ Business (PCBU) Person/s responsible for Workers - It is your responsibility to; provide adequate supervision of your workers, review the Risk Assessment (JHA, SWMS) if the scope of work changes.
